Concerns Botox and Xeomin Can Treat

Botox and Xeomin are most commonly used to soften wrinkles caused by repeated facial-muscle movement. These dynamic lines may initially appear only with expression and become more visible at rest over time.

Common treatment concerns include:

  • Vertical frown lines between the eyebrows, sometimes called the “11s”

  • Horizontal forehead lines

  • Crow’s feet around the outer corners of the eyes

BOTOX Cosmetic is FDA approved for the temporary improvement of moderate to severe frown lines, forehead lines, crow’s feet, and vertical platysma bands connecting the neck and jaw in adults. XEOMIN is FDA approved for the temporary improvement of moderate to severe upper facial lines, including frown lines, forehead lines, and crow’s feet.

Other aesthetic uses—such as a lip flip, gummy-smile treatment, chin dimpling, downturned mouth corners, bunny lines, and masseter treatment—are considered off-label. Off-label prescribing is common in medicine, but these treatments require careful anatomical assessment and an individualized discussion of potential benefits and risks.

 

Dynamic Wrinkles Versus Lines at Rest

Neuromodulators work best for wrinkles produced or intensified by muscle contraction. If a line remains deeply visible when the face is completely relaxed, reducing muscle movement may soften it without removing it entirely.

Static lines, skin laxity, volume loss, and texture changes may require a combination approach. Depending on the concern, we may discuss VirtueRF microneedling, SkinPen microneedling, dermal fillers, biostimulators, or other skin treatments.

The goal is to match the treatment to the cause of the concern rather than assuming that every line should be treated with more neurotoxin.

 

Botox Versus Xeomin

BOTOX Cosmetic contains onabotulinumtoxinA. XEOMIN contains incobotulinumtoxinA. Both temporarily reduce muscle activity by limiting acetylcholine release at the neuromuscular junction, but they are manufactured and formulated differently.

XEOMIN is produced without accessory complexing proteins. This difference does not make one product universally better than the other. Product selection may depend on the treatment area, facial anatomy, previous response, treatment goals, provider recommendation, and patient preference.

The potency units of Botox and Xeomin—and of all botulinum-toxin products—are product specific and are not directly interchangeable. A patient’s previous number of units with another product should not automatically be used as the dose for a new product.

Who Is an Appropriate Candidate?

Appropriate candidates are adults with dynamic facial lines, platysma bands, or selected lower-face concerns who understand that neuromodulator results are temporary. Good candidates have realistic expectations and are willing to discuss the balance between wrinkle reduction and preservation of facial expression.

Treatment may not be appropriate for patients with:

  • Infection, inflammation, or open skin at a proposed injection site

  • A known hypersensitivity to the selected botulinum-toxin product or its ingredients

  • Certain neuromuscular disorders, including myasthenia gravis or Lambert-Eaton syndrome

  • Significant baseline swallowing or breathing difficulties

  • Marked weakness or atrophy in the proposed treatment muscle

  • An expectation that neurotoxin will correct substantial volume loss or skin laxity

Additional caution may be necessary for patients with pre-existing eyelid or eyebrow drooping, significant facial asymmetry, prior facial surgery, altered facial anatomy, dry-eye symptoms, bleeding disorders, or medications that affect neuromuscular transmission.

Elective cosmetic treatment is generally deferred during pregnancy and breastfeeding because adequate safety data are not available. Patients should also report any botulinum-toxin product received within the previous four months, even if it was administered at another office or for a medical indication.

 

What Happens During Treatment?

Your appointment begins with a facial assessment at rest and during movement. You may be asked to frown, raise your eyebrows, smile, squint, purse your lips, contract your chin, or tense your neck so the provider can evaluate the muscles producing the concern.

The provider will review your goals, previous treatments, medications, allergies, medical history, and baseline asymmetries. The planned injection sites and product will then be discussed before consent.

After the skin is cleansed, small amounts of Botox or Xeomin are injected into selected muscles using a fine needle. The number of injection sites and units depends on the treatment area, muscle strength, facial proportions, sex, previous response, and desired degree of movement reduction.

The injections commonly take approximately 10 to 20 minutes. Most patients describe a brief pinch or pressure. Topical anesthesia is usually unnecessary, although comfort measures may be used when appropriate.

 

How Many Units Will I Need?

There is no single appropriate number of units for every patient. A larger dose is not automatically better, and matching a friend’s dose does not ensure a similar result.

Dose selection depends on:

  • Product being used

  • Number of areas treated

  • Muscle size and strength

  • Facial anatomy and eyebrow position

  • Baseline asymmetry

  • Sex and muscle mass

  • Previous treatment response

  • Desired preservation of movement

  • Risk of eyebrow, eyelid, lip, or smile changes

An exact treatment plan can only be determined after an in-person assessment. Botox and Xeomin units should never be compared as though all neurotoxin products use an identical unit scale.

 

When Will I See Results?

Early effects may become noticeable within the first several days. The response generally continues to develop over the following one to two weeks. Manufacturer clinical results are often formally assessed later, including at approximately 30 days, but most patients recognize a meaningful change before then.

A follow-up assessment may be scheduled at approximately two weeks, particularly for a first treatment, a new treatment area, or a patient with substantial baseline asymmetry. Evaluating too early may not reflect the final response.

 

How Long Do Results Last?

Results commonly last approximately three to four months, although duration varies. Some patients metabolize the effect sooner, while others retain improvement longer.

Duration may be influenced by:

  • Treatment area

  • Muscle strength

  • Dose and injection pattern

  • Product selected

  • Individual metabolism

  • Exercise patterns

  • Previous treatment history

  • Degree of movement the patient wants to preserve

Routine maintenance treatments are generally spaced at least 90 days apart. A scheduled follow-up is different from automatically repeating an entire treatment. At follow-up, the provider determines whether the result is balanced and whether any adjustment is clinically appropriate.

 

Downtime and Aftercare

Botox and Xeomin usually require little functional downtime. Small bumps at injection sites commonly resolve quickly. Mild redness, tenderness, swelling, headache, or bruising may occur.

After treatment:

  • Do not rub, massage, or apply firm pressure to the treated areas unless specifically instructed.

  • Avoid facials, facial massage, or other procedures that place pressure on the injection sites immediately after treatment.

  • Follow your provider’s instructions regarding vigorous exercise, heat exposure, and other activities for the remainder of the day.

  • Do not start a new medication without telling the prescribing professional that you recently received botulinum toxin.

  • Contact Modern Medical Spa if you develop an unexpected or concerning symptom.

Do not stop medically necessary aspirin, anticoagulants, or other prescribed medication solely to reduce bruising unless the prescribing clinician has approved the change.

 

Potential Risks and Side Effects

Common or localized reactions may include injection-site discomfort, bruising, redness, swelling, headache, temporary tenderness, dry eye, watery eye, or temporary asymmetry.

Depending on the area treated, unintended muscle weakening may cause:

  • Eyelid or eyebrow drooping

  • Brow heaviness

  • An uneven smile

  • Lip weakness

  • Difficulty using a straw

  • Changes in speech or oral competence

  • Neck weakness

  • Difficulty swallowing

  • An undesired change in facial expression

Botulinum-toxin products carry a boxed warning concerning possible spread of toxin effect beyond the injection site. Rare but serious symptoms can include generalized weakness, double or blurred vision, hoarseness, difficulty speaking, loss of bladder control, difficulty swallowing, or difficulty breathing. These symptoms may occur hours to weeks after treatment and require immediate medical attention.

Serious allergic reactions are also possible. Seek emergency care for difficulty breathing, wheezing, facial or throat swelling, faintness, or other symptoms suggesting a severe allergic reaction.

 

Masseter Botox and Jawline Slimming

Botox or Xeomin may be injected off-label into enlarged masseter muscles to reduce excessive lower-face width caused by muscle bulk. This treatment does not remove fat, tighten loose skin, correct bone structure, or replace evaluation of significant jaw pain or dental disease.

Masseter treatment requires careful dosing because these muscles are involved in chewing and help support lower-face structure. Potential adverse effects include chewing fatigue, smile asymmetry, changes in facial contour, or increased visibility of lower-face laxity in some patients.

Patients seeking improvement in jawline definition may need a different or combined approach. Depending on the cause, options could include dermal filler, a collagen-stimulating biostimulator, or another facial-contouring treatment.

 

Lip Flip Versus Lip Filler

A lip flip uses a small amount of neurotoxin around the upper lip to reduce inward rolling of the lip during smiling. It may create a subtle increase in visible upper-lip height but does not add volume.

Lip filler uses hyaluronic-acid gel to add or shape volume. Patients seeking a noticeable increase in lip fullness will generally require filler rather than a lip flip. Some patients may be candidates for both treatments, but the benefits, risks, timing, and expected degree of change are different.

 

Combining Neurotoxins With Other Treatments

Neuromodulators may be incorporated into a broader facial-treatment plan when more than one process contributes to the patient’s concern.

Can I exercise after Botox or Xeomin?

Your provider will give specific instructions. Vigorous exercise may be restricted for the remainder of the treatment day. Avoid rubbing, massaging, or placing firm pressure over the injection sites.

 

Can I receive Botox or Xeomin before an important event?

Can Botox be combined with filler or microneedling?
